Pennsylvania Set Up
Pennsylvania MVRs are currently available via faxed return from First Advantage/ADR. To gain access to the Pennsylvania database, you must first complete a one-page, state form titled "Affidavit of Intended Use" (copy and instructions below) and provide ExamOne a copy of your company's "Certificate of Authority" (COA) issued by PA (usually kept in your legal or compliance area).
In order to get the code, you must send both the Affidavit and the COA as soon as possible. You must have at least one officer's complete data shown in the boxes under "Ownership." ExamOne will review your paperwork for completeness to avoid delays in getting your number. After you have completed the form and copied the COA, you will need to fax a copy of each immediately to ExamOne then mail the original Affidavit and the copy of the COA to us thereafter. Be sure that the Affidavit is notarized and that the date of notarizing is the same as the date the form was signed.
Additionally, the result will no longer be coming from ExamOne to you. First Advantage/ADR, our state interface for PA (only PA), has determined that PA state law precludes our ever seeing the MVR result. Therefore, we will need to provide First Advantage/ADR with the fax number you want the result to go back to. Please include that fax number to us as well.
In short, we need from you:
- A fully completed, notarized PA "Affidavit of Intended Use" (faxed and original mailed). >> Download Affidavit
- A photocopy of the PA "Certificate of Authority" for conducting business in the state (faxed and mailed).
3. The fax number that you want results sent back to.
Pennsylvania typically takes three to four weeks to complete the set-up process. Once your paperwork has been forwarded to Pennsylvania and has been processed, they will provide ExamOne with a Pennsylvania Requestor Code. ExamOne will ensure that this code is appended to all of your orders that are sent to this state thereafter. We will notify you when you may begin ordering.
